As a Services Coordinator…
You will work with individuals who are eligible for case management services through Intellectual and Developmental Disabilities (I/DD) and their family/support team.
As a Service Coordinator, you will provide case management services for I/DD individuals and their families and assist them in accessing services and resources. You will be responsible for developing, coordinating, and monitoring Individual Support Plans (ISPs).
You will work with partnering agencies, such as Aging and People with Disabilities, local education associations and school districts, law enforcement, the courts, community organizations that provide services to individuals experiencing intellectual and developmental disabilities and clinicians to provide comprehensive service coordination to individuals enrolled in DD services
You will serve as an advocate/liaison to assist clients obtain necessary services, which may include referrals and finding resources in the community.
Average Wage Range
$50,000-$70,000 annual salary
To become a Service Coordinator...
As a Service Coordinator, you will possess any of the following education and/or experience:
- A Bachelor's degree in behavioral science, social science, or a closely related field; or
- A Bachelor's degree in any field and one year of human services related experience, such as work providing assistance to individuals and groups with issues such as economic disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or
- An Associate's degree in a behavioral science, social science, or a closely related field and two years of human services related experience.
- Three years of human services related experience.
Services Coordinator's must possess and maintain a valid Oregon driver's license, and have access to and use of a personal automobile as a condition of employment.
